Making a fatality claim

If you have lost a family member in a motor vehicle accident.

Accessing MAC support

To access support from MAC, you will need to complete and submit a fatality claim form. This will help us assess your entitlements and the benefits that may be available.

We will support you through the process for claiming benefits and let you know how to complete the necessary forms and the information you may need to provide.

If you need any help completing the claim form, please call the TIO MAC claims team on 1300 493 506 during business hours.

When to make a claim

A MAC claim can be made immediately following the death of a loved one so that we can provide support through this extremely difficult time.

Who can make a claim

The next of kin, close family member or guardian can make a MAC claim when a family member dies.

You can make a claim by downloading and completing a fatality claim form. Fatality claim forms are also available from hospitals in the Northern Territory. If you would like a claim form sent to you in the mail, please call the TIO MAC claims team on 1300 493 506 between 8am- 5pm Monday to Friday.

To help us process your claim as quickly as possible, please include all the details required on the claim form.

When lodging the claim, you should also provide evidence of any dependants (spouse or dependent children) such as a marriage and birth certificates or financial documents so that we can assess any entitlements for dependants.

The Coroner’s report

The Coroner is a Government official who investigates unexpected deaths and the circumstances around the cause of death and prepares a report.

More information about the Coroner can be found here Introduction | NT.GOV.AU

When available, a Coroner’s report that confirms the circumstances around the death is necessary, so that we can assess what financial benefits may be available for surviving dependants.
