If you have lost a family member in a motor vehicle accident.
To access support from MAC, you will need to complete and submit a fatality claim form. This will help us assess your entitlements and the benefits that may be available.
We will support you through the process for claiming benefits and let you know how to complete the necessary forms and the information you may need to provide.

A MAC claim can be made immediately following the death of a loved one so that we can provide support through this extremely difficult time.
The next of kin, close family member or guardian can make a MAC claim when a family member dies.
You can make a claim by downloading and completing a
fatality claim form. Fatality claim forms are also available from hospitals in the Northern Territory. If you would like a claim form sent to you in the mail, please call the TIO MAC claims team on

between 8am- 5pm Monday to Friday.
To help us process your claim as quickly as possible, please include all the details required on the claim form.
When lodging the claim, you should also provide evidence of any dependants (spouse or dependent children) such as a marriage and birth certificates or financial documents so that we can assess any entitlements for dependants.
The Coroner is a Government official who investigates unexpected deaths and the circumstances around the cause of death and prepares a report.

When available, a Coroner’s report that confirms the circumstances around the death is necessary, so that we can assess what financial benefits may be available for surviving dependants.

After we have received your completed claim form, you will receive written notification from TIO MAC which will include your claim number and the name and contact details of your MAC case manager.
Your case manager will be your first contact for managing your claim. They will call you to introduce themselves, answer any questions you may have and guide you through the claims process. If there is any further information we require, your case manager will let you know.
